摘要:
目的 探讨2型糖尿病(T2DM)患者血栓弹力图(TEG)参数与铁代谢指标的相关性。方法 选取2012年10月—2014年4月在海口市人民医院门诊就诊的T2DM患者102例(T2DM组),根据病程分为两个亚组:病程≤5年组(n=49)和病程>5年组(n=53)。另选择98名健康者作为对照组。检测并比较两组红细胞铁、血清铁、血清铁蛋白、铁调素、转铁蛋白及TEG。结果 T2DM组的红细胞铁、血清铁、血清铁蛋白、铁调素及转铁蛋白均明显高于对照组(t=12.33~63.77,P均<0.01), 病程>5年组转铁蛋白较病程≤5年组升高(t=0.155,P<0.01); 病程>5年组R值(反应时间)、K值(凝固时间)低于对照组和病程≤5年组(F=23.48、29.56,P均<0.01),α角(凝固角)、MA值(血凝块最大强度)高于对照组和病程≤5年组(F=11.85、17.71,P均<0.01); 病程>5年组CI值(综合凝血指数)较对照组增加(F=18.23,P<0.01)。Pearson相关分析显示,病程>5年组血清铁蛋白与TEG参数K值及α角呈正相关(r=0.107、0.230,P均<0.01); 转铁蛋白与TEG参数R值、K值及α角呈负相关(r=-0.259,-0.399,-0.241,P均<0.01),与MA值、CI值呈正相关(r=0.168、0.258,P<0.05和P<0.01)。结论 T2DM患者存在以红细胞铁、血清铁、血清铁蛋白、铁调素、转铁蛋白升高为主的铁超负荷,且TEG检测的多项参数与铁代谢指标存在相关性。TEG可作为T2DM患者凝血功能监测的一种手段。
Abstract:
Objective To investigate the correlation between iron metabolism index and thrombelastography(TEG)parameter in patients with type 2 diabetes mellitus(T2DM).Methods A total of 102 outpatients with T2DM at Haikou People's Hospital from October 2012 to April 2014 were selected(T2DM group)and divided into sub-group: diabetic duration≤5 years group(n=49)and duration >5 years group(n=53); another 98 healthy people were chosen as control group.Erythrocyte ferritin, serum iron, serum ferroprotein, hepcidin, transferrin, and parameter of TEG were detected and compared between two groups. Results The level of erythrocyte ferritin,serum iron, serum ferroprotein, hepcidin, transferrin in T2DM group were significant higher than those in control group(t=12.33-63.77, all P<0.01). The transferrin level was higher in duration>5 years group than that in duration ≤ 5 years group(t=0.155, P<0.01). Compared with duration>5 years group, the values of response time(R)and clot time(K)were decreased(F=23.48,29.56, all P<0.01), whereas the α angle and maximum clot strength(MA)were increased(F=11.85,17.71, all P<0.01)in duration ≤ 5 years group and control group. Clot index(CI)in duration >5 years group was higher than that in control group(F=18.23, P<0.01). Pearson correlation analysis showed that the level of serum iron was positively correlated with the values of R, K and α angle(r=0.107,0.230, all P<0.01). Transferrin was negatively correlated with the values of R, K and α angle(r=-0.259,-0.399,-0.241, all P<0.01)and positively correlated with the values of MA and CI(r=0.168,0.258, P<0.05 and P<0.01)in duration>5 years group. Conclusions Iron overload such as ferritin,serum iron,serum ferroprotein, transferrin, hepcidin exists in patients with T2DM.Parameters of TEG are related with the level of iron metabolism index. TEG technology can be used as an effective method to monitor coagulation status of patients with T2DM.
