摘要:
目的 探讨2型糖尿病(T2DM)患者血清Apelin、血管紧张素Ⅱ(Ang Ⅱ)水平与颈动脉内膜中层厚度(CIMT)的相关性。方法 选取2018年1月—2018年6月于蚌埠医学院第一附属医院住院的T2DM患者103例,根据CIMT分组,CIMT<0.9 mm为NCIMT组(n=48),CIMT>0.9 mm为ACIMT组(n=55),另外选取49名同期健康体检者为正常对照组(NC组)。所有研究对象均计算体重指数、腰臀比; 记录收缩压、舒张压; 检测空腹血糖、HbA1c、总胆固醇、甘油三酯等生化指标。采用酶联免疫吸附法测定血清Apelin、AngⅡ浓度。比较3组之间上述指标的差异,偏相关分析Apelin及Ang Ⅱ的相关因素,logistic二元回归分析CIMT的影响因素。结果 与NC组相比,NCIMT组血清 Apelin浓度升高,与ACIMT组相比,NCIMT组血清Apelin降低(F=17.768,P<0.01)。与NC组相比, NCIMT组血清Ang Ⅱ浓度升高,与NCIMT组相比,ACIMT组血清Ang Ⅱ浓度升高(F=8.727,P<0.01)。偏相关分析显示,血清 Apelin浓度与腰围、腰臀比、总胆固醇、空腹血糖、HbA1c呈正相关(r=0.223、0.238、0.271、0.227、0.580,P均<0.05); 血清Ang Ⅱ浓度与收缩压、HbA1c、总胆固醇呈正相关(r=0.190、0.459、0.267,P均<0.05)。Logistic二元回归分析显示,年龄、HbA1c、Ang Ⅱ与CIMT呈正相关(OR=1.087,95%CI:1.033~1.145,P=0.001; OR=1.986,95%CI:1.189~3.316,P=0.009; OR=1.021,95%CI:1.009~1.033,P=0.000),血清Apelin浓度和CIMT呈负相关(OR=0.098,95%CI:0.997~0.999,P=0.004)。结论 血清Apelin是CIMT增厚的保护因素,血清Ang Ⅱ是CIMT增厚的危险因素,可能与T2DM患者动脉粥样硬化有关。
Abstract:
Objective To explore the relationship between serum level of Apelin, angiotensin(Ang)Ⅱ and carotid intima-media thickness(CIMT)in patients with type 2 diabetes mellitus(T2DM).Methods A total of 103 patients with T2DM admitted to the First Affiliated Hospital of Bengbu Medical College from January 2018 to June 2018 were selected. All patients were divided into two groups based on CIMT: CIMT<0.9 mm(NCIMT group, n=48)and CIMT>0.9 mm(ACIMT group, n=55). In addition, 49 healthy subjects underwent physical examination during the same time were selected as the normal control group(NC group, n=49). Body mass index(BMI)and waist-hip ratio(WHR)were calculated; systolic blood pressure(SBP)and diastolic blood pressure(DBP)were recorded; fasting glucose(FPG), HbA1c, total cholesterol(TC)and triglyceride(TG)were measured for all the selected individuals. Meanwhile, enzyme-like immunosorbent assay was used to determine fasting serum Apelin and Ang Ⅱ levels. Differences of these indicators were compared among the three groups. The related factors of Apelin and Ang Ⅱ were analyzed by Pearson correlation analysis, and the influential factors of CIMT were analyzed by logistic binary regression.Results Compared with NC group, serum level of Apelin in NCIMT group was increased; compared with ACIMT group, serum level of Apelin in NCIMT group was decreased(F=17.768, P<0.01). Compared with NC group, serum level of Ang Ⅱ was increased in NCIMT group, and compared with NCIMT group, serum level of Ang Ⅱ was increased in ACIMT group(F=8.727, P<0.01). Pearson correlation analysis showed that serum level of Apelin was positively correlated with waist circumference, WHR, TC, FPG and HbA1c(r=0.223, 0.238, 0.271, 0.227, 0.580, respectively, all P<0. 05). Serum level of Ang Ⅱ was positively correlated with SBP, HbA1c and TC(r=0.190, 0.459, 0.267, all P<0.05). Logistic binary regression analysis showed that age, HbA1c and Ang Ⅱ were positively correlated with CIMT(OR=1.087, 95%CI:1.033-1.145, P=0.001; OR=1.986, 95%CI:1.189-3.316, P=0.009; OR=1.021, 95%CI: 1.009-1.033, P=0.000), while Apelin was negatively correlated to CIMT(OR=0.098, 95%CI: 0.997-0.999, P=0.004).Conclusion Serum Apelin is a protective factor for CIMT, and Ang Ⅱ is a risk factor for CIMT and may be associated with the development of atherosclerosis in patients with T2DM.
