摘要:
目的 通过脑干听觉诱发电位(BAEP)检查探讨2型糖尿病(T2DM)患者脑干听觉通路异常与糖尿病视网膜病变(DR)的相关性。方法 选取哈尔滨医科大学附属第一医院收治的T2DM患者467例,收集患者一般临床资料、血糖、血脂、血压、肝功能、肾功能等,对其进行BAEP及糖尿病相关并发症检查。以有无DR作为分组依据,比较两组间BAEP结果的差异并将BAEP结果与DR严重程度做相关性分析。结果 两组间糖化血红蛋白A1c(HbA1c)、Ⅲ 波峰间期(PL)、PL Ⅴ、Ⅰ~Ⅴ波峰间潜伏期(IPL)、糖尿病肾脏病(DKD)、糖尿病神经病变(DPN)、下肢动脉硬化(LEAD)差异有统计学意义(P均<0.05)。BAEP与DR严重程度显著相关,BAEP与logistic回归分析结果显示,双耳PL Ⅲ、PL Ⅴ、IPL Ⅰ~Ⅴ延长,DR的风险也随之升高。结论 T2DM患者脑干听觉通路异常与DR严重程度相关。
Abstract:
Objective The correlation between abnormal brainstem auditory pathway and diabetic retinopathy(DR)in patients with type 2 diabetes mellitus(T2DM)was investigated by brainstem auditory evoked potential(BAEP).Methods A total of 467 patients with T2DM in the First Affiliated Hospital of Harbin Medical University were selected. The general clinical data, blood glucose, blood lipids, blood pressure, liver function and renal function of the patients were collected, and BAEP and diabetes-related complications were examined. Based on the presence or absence of DR, the differences in BAEP between the two groups were compared and the correlation between BAEP and the severity of DR was analyzed.Results There were statistical differences in glycated hemoglobin A1c(HbA1c), Ⅲ peak interval(PL), PL Ⅴ, Ⅳ peak latency(IPL), diabetic kidney disease(DKD), neuropathy(DPN), and lower extremity arteriosclerosis(LEAD)between the two groups(all P<0.05). BAEP was significantly correlated with the severity of DR. The results from logistic regression analysis showed that the prolongation of binaural PL Ⅲ, PL Ⅴ, and IPL Ⅰ~Ⅴ increased the risk of DR.Conclusion Abnormal brainstem auditory pathway is associated with the severity of DR in patients with T2DM.
