摘要:
目的 观察男性2型糖尿病患者血尿酸与性激素水平的关系。方法 选择男性2型糖尿病患者112例,将患者的血尿酸水平由低到高排列并分成4等份,处于3个分割点位置的数值为251,290,385 μmol/L,按四分位法进行分组(Q1组,Q2组,Q3组,Q4组)。所有患者测定血糖、血尿酸、睾酮、性激素结合球蛋白(SHBG)、孕激素、催乳素、黄体生成素(LH)、卵泡刺激素、雌二醇、脱氢表雄酮,并计算得出游离睾酮、生物活性睾酮(BT),比较4组间性激素水平与血尿酸水平差异,对睾酮、游离睾酮、BT及SHBG与尿酸进行相关性分析。结果 与Q1组相比,Q4组空腹胰岛素水平及餐后胰岛素水平显著升高(F=16.141,Z=30.85,P均<0.05),总睾酮、SHBG水平显著降低(F分别为3.810,2.783,P均<0.05)。相关性分析显示,SHBG与血尿酸呈负相关(r=0.24,P<0.05)。结论 男性2型糖尿病患者存在性腺功能减退症,并与血尿酸水平相关。
Abstract:
Objective To explore the relationship between blood uric acid and sex hormones in male patients with type 2 diabetes. Methods A total of 112 male patients with type 2 diabetes were divided into four groups(group Q1,group Q2,group Q3,group Q4)according to serum uric acid level using the quarterback method with the three cutting points of 251, 290, or 385 μmol/L, respectively.Blood glucose, blood uric acid, testosterone, sex hormone binding globulin(SHBG), progesterone, prolactin, luteinizing hormone(LH), follicle stimulating hormone, estradiol, dehydroepiandrosterone were detected.Free testosterone and bioavailable testosterone(BT)were calculated. The level of sex hormones and blood uric acid were compared between 4 groups. The correlations between testosterone, free testosterone, BT, SHBG and uric acid were analyzed. Results Compared with group Q1, group Q4 had a higher level of fasting insulin(F=16.141,P<0.05)and postprandial insulin(Z=30.850,P<0.05), but a lower level of estradiol and SHBG(F=3.810,2.783, all P<0.05). Correlation analysis showed that SHBG was negatively correlated with blood uric acid(r=0.24,P<0.05).Conclusion Male patients with type 2 diabetes have hypogonadism, and it is related to blood uric acid level.
