摘要:
原发性甲状旁腺功能亢进症(PHPT)的治疗除了传统的手术和药物之外,以微波消融、射频消融为主的热消融技术用于治疗PHPT的报道越来越多。现有临床研究表明,热消融可以显著降低PHPT患者甲状旁腺激素和血钙水平。同时,热消融治疗PHPT具有良好的安全性,少数患者出现的微小并发症,均可自行缓解。受限于临床研究的质量和数量,目前对PHPT热消融的适应证、术中管理、术后随访和疗效评价仍无统一认识。未来仍需要开展设计严谨、样本量较大的临床试验,以进一步评估热消融治疗PHPT的有效性和安全性,以提高PHPT治疗的管理质量。
Abstract:
Apart from surgery and medication as traditional therapies for primary hyperparathyroidism(PHPT), emerging studies focused on thermal ablation to treat PHPT, especially microwave ablation and radiofrequency ablation. Currently, clinical researches have proved that levels of parathyroid hormone and serum calcium significantly dived after PHPT patients received thermal ablation compared with those before treatment. Besides, the safety of thermal ablation has been clarified. A small number of patients developed minor complications that could be spontaneously relieved with time. However, limited by the quality and quantity of clinical studies, there is no consensus on the indications, peri-ablative management, post-ablative follow-up and efficacy assessment of thermal ablation in the treatment of PHPT. In the future, clinical trials with refined design and larger sample size are still needed to further evaluate the effectiveness and safety of thermal ablation for PHPT in order to improve the management quality of PHPT.
