摘要:
目的 观察不同程度阻塞性睡眠呼吸暂停低通气综合征(OSAHS)患者骨密度变化及与胰岛素样生长因子-1(IGF-1)的关系。方法 根据呼吸暂停低通气指数(AHI)将46例男性OSAHS患者分为3组:轻度OSAHS组(5次/h≤AHI<15次/h)16例、中度OSAHS组(15次/h≤AHI<30次/h)16例、重度OSAHS组(AHI≥30次/h)14例。选择健康男性体检者16名为正常对照组。用化学发光法测定IGF-1水平、双能X线骨密度仪测定骨密度,进行组间比较及与AHI的相关性分析。结果 与正常对照组相比,中、重度OSAHS患者腰椎及股骨颈骨密度降低(F=19.614、22.337,P均<0.05); OSAHS患者IGF-1水平显著降低,且随着病情加重逐渐降低,差异均有统计学意义(F=39.615,P=0.000)。对于OSAHS患者,IGF-1与AHI呈负相关(r=-0.301,P<0.01),体重指数与AHI呈正相关(r=0.269,P<0.05)。IGF-1与AHI呈负相关(r=-0.301,P<0.01)。结论 OSAHS患者骨密度降低,IGF-1可能在其骨代谢变化中起重要作用。
Abstract:
Objective To explore the changes of bone mineral density and its relationship between insulin-like growth factor-1(IGF-1)in patients with different degree of obstructive sleep apnea hypopnea syndrome(OSAHS). Methods A total of 46 male patients with OSAHS were divided into three groups based on the apnea hypopnea index(AHI):16 patients in mild OSAHS group(5 times/h≤AHI<15 times/h), 16 patients in moderate OSAHS group(15 times/h≤AHI<30 times/h), 14 patients in severe OSAHS group(AHI≥30 times/h). Another 16 male health people undertaken medical examination were also recruited as normal control group. Serum IGF-1 was examined with chemiluminescence assay. Bone mineral density was examined with dual energy X ray absorptiometry. The results were compared among groups and their correlation with AHI was analyzed. Results Compared with normal control group, bone mineral density of lumbar spine and femoral neck in mild and severe OSAHS group were significantly lower(F=19.614,22.337,all P<0.014). The level of IGF-1 in different OSAHS groups was significantly decreased,which was gradually decreased with the aggravation of OSAHS(F=39.615,P=0.000). The correlation between IGF-1 and AHI was negative(r=-0.301, P<0.001), correlation between body mass index and AHI was positive(r=0.269, P<0.05), and IGF-1 was inversely associated with AHI(r=-0.301,P<0.01). Conclusion Bone mineral density of patients with OSAHS is decreased,and IGF-1 may play an important role in the changes of bone metabolism.
