摘要:
目的 探讨不同间歇性禁食对超重、肥胖患者的临床疗效及优势。方法 通过检索中英文数据库间歇性禁食治疗超重、肥胖患者的随机对照试验,进行网状荟萃分析。结果 本研究共纳入18篇文献,网状荟萃分析结果显示:通过SUCRA排序,限时禁食(TRF)在改善体重指数、体重、腰围、脂肪重量以及脱落率指标上成为最佳干预措施的可能性最大,次之为隔日禁食(ADF); 对于总胆固醇、空腹血糖、胰岛素抵抗控制,ADF表现更佳,次之为TRF; 结合可信区间及两两对比分析,TRF在不同指标上并不劣于ADF,且在脱落率上优于ADF; 本研究结果无明显不一致性,但存在一定的小样本效应或发表偏倚。结论 相对于持续性能量限制饮食,TRF及ADF改善超重肥胖患者脱落率、肥胖及代谢指标效果更佳; 体重指数及脱落率指标上,TRF具有优于ADF的可能性,而对于腰围、体重、脂肪重量、空腹血糖、总胆固醇、胰岛素抵抗指标,TRF并不劣于ADF,因此,TRF更适合依从性较差的超重、肥胖人群,但仍需临床试验进一步证实。
Abstract:
Objective To investigate the clinical efficacy and advantages of different intermittent fasts in overweight or obese patients.Methods A net meta-analysis was performed by searching English and Chinese databases of randomized controlled trials of intermittent fasting for the treatment of overweight and obese patients.Results A total of 18 papers were included in this study, and the results of the reticulated Meta-analysis: by SUCRA ranking, time-restricted fasting(TRF)was the most likely to be the best intervention in terms of improving BMI, weight, waist circumference, fat weight, and shedding rate indicators, followed by alternate-day fasting(ADF); for total cholesterol, fasting glucose, and insulin resistance control, ADF performed better, followed by TRF. Combined with the confidence interval and two-comparison analysis, TRF was not inferior to ADF in different indicators and was better than ADF in shedding rate; the results of this study were not significantly inconsistent but there was some small sample effect or publication bias.Conclusion Compared with continuous energy-restricted diet, time-restricted fasting and alternate-day fasting are more effective in improving shedding rate, obesity and metabolic indexes in overweight and obese patients; for BMI and shedding rate indexes, time-restricted fasting is more likely than alternate-day fasting, while for waist circumference, body weight, fat weight, fasting glucose, total cholesterol and insulin resistance indexes, time-restricted fasting is not inferior to alternate-day fasting, therefore, time-restricted fasting is more suitable for overweight obese patients with poor compliance, but further confirmation in clinical trials is needed.
