摘要:
目的 探究江西省居民中国人内脏脂肪指数(CVAI)与糖代谢异常患病的关系。方法 该研究基于2018年江西省成人慢性病及其危险因素监测调查结果,用多元logistic回归分析,分析不同性别、不同CVAI水平(四分位数分组)纳入者与糖尿病前期、糖尿病患病的关系。结果 糖耐量正常组2 996名,糖尿病前期组1 097名,糖尿病组697例。3组研究对象中不同CVAI水平组居民糖代谢异常患病分布差异有统计学意义(P<0.001),且糖尿病前期和糖尿病患病率均随CVAI的增长呈上升趋势,且女性较男性增长明显。多元logistic回归分析结果表明,糖尿病前期和糖尿病的患病率随着CVAI水平的升高而增加(P均<0.05),在校正年龄后,CVAI仍与糖代谢状态显著相关,其中男性CVAI最高和最低四分位数之间的比较中,糖尿病前期和糖尿病的优势比(95% CI)分别为2.40(1.66~3.46)、2.76(1.83~4.16),女性CVAI最高和最低四分位数之间的比较中,糖尿病前期和糖尿病的OR(95% CI)分别为4.81(3.45~6.72)、7.38(4.84~11.26),差异有统计学意义(P均<0.05); 再校正相关混杂因素后,女性CVAI最高和最低四分位数之间的比较中,糖尿病前期和糖尿病的OR(95% CI)分别为2.23(1.14~4.38)、3.87(1.73~8.61),差异有统计学意义(P均<0.05)。结论 CVAI与糖代谢异常密切相关,且女性较男性显著。CVAI值越大,糖尿病前期和糖尿病患病风险越高,对糖代谢异常患病的预测有重要价值。
Abstract:
Objective To investigate the relationship between Chinese visceral adiposity index(CVAI)and abnormal glucose metabolism among residents of Jiangxi province.Methods This study was based on the survey results of adult chronic diseases and their risk factors in Jiangxi province in 2018.Multivariate logistic regression analysis was used to analyze the relationship between different genders, different CVAI levels(quartile grouping)and prediabetes and diabetes.Results There were 2 996 patients in the normal glucose tolerance group, 1 097 patients in the pre-diabetes group, and 697 patients in the diabetes group. In the 3 groups,there was a statistically significant difference in the distribution of abnormal glucose metabolism among residents of different CVAI level groups(P<0.001), and the pre-diabetes and diabetes prevalence rates both increased with the increase of CVAI, and the increase in women was more obvious than that of men. The results of multivariate logistic regression analysis showed that with the increase of CVAI level, the pre-diabetes and diabetes prevalence rates also increased(all P<0.05). After adjusting for age, CVAI was still significantly correlated with glucose metabolism status, and male CVAI was the highest in the comparison with the lowest quartile, the OR(95% CI)of prediabetes and diabetes were 2.40(1.66-3.46)and 2.76(1.83-4.16), respectively; In the comparison between the highest and lowest quartile of female CVAI, the OR(95% CI)of prediabetes and diabetes were 4.81(3.45-6.72)and 7.38(4.84-11.26), respectively, and the differences were statistically significant(all P<0.05); After correcting for related confounding factors, the comparison between the highest and lowest quartile of female CVAI, the OR of prediabetes and diabetes(95% CI)were 2.23(1.14-4.38)and 3.87(1.73-8.61), respectively, the differences were statistically significant(all P<0.05).Conclusion CVAI is closely related to abnormal blood glucose metabolism, and it is more significant in women than men. The greater the CVAI value, the higher the risk of pre-diabetes and diabetes, and it has important clinical value for the prevention and diagnosis of abnormal glucose metabolism.
