摘要:
既往研究认为大多数胰岛素瘤的发生与多发性内分泌腺瘤病1型基因及其产物menin蛋白有关,而近年来相继发现新的基因如YY1(Yin Yang1)也参与其中。由于大多数瘤体直径<1 cm,临床上很难进行定位诊断。68镓(68Ga)标记胰高血糖素样肽-1受体激动剂的正电子发射计算机断层显像(PET-CT)检查有助于提高阳性检出率。目前外科手术仍是胰岛素瘤的首选治疗方法,对于高龄或者不适合手术的患者,超声内镜介导的乙醇消融法可作为治疗选择。
Abstract:
Multiple endocrine neoplasia 1(MEN1) gene and its coding protein--menin were firstly found associated with insulinoma,whereas many new genes have been discovered to be associated with insulinoma,such as YY1(Yin Yang1). Since the diameters of most insulinomas are smaller than 1 cm,it is too difficult to locate the tumor with conventional methods. Positron emission tomography (PET) scan with glucagon-like peptide 1 receptor agonists labeled by 68Gallium(68Ga)has been shown efficient in improving the tumor detecting rate. Surgical resection is still a preferred treatment for insulinoma. However Eus-guided ethanol ablation may be an alternative therapy for some patients,especially for aged people.
