摘要:
目的 探讨血清、滑液中白细胞介素-1β(interleukin-1β, IL-1β)和环氧化酶-2(cyclooxygenase -2, COX-2)在痛风性关节炎(gouty arthritis,GA)中的动态变化及临床意义。方法 选择2016年1月至2019年12月于徐州市中医院门诊或住院诊治的80例GA患者作为观察组,其中缓解期(Ⅰ组)29例,急性发作期轻度(Ⅱ组)31例、重度(Ⅲ组)20例,另选取30名健康志愿者作为对照组。比较各组常规实验室指标和血清中IL-1β、COX-2水平; Spearman相关性分析IL-1β、COX-2与尿酸(uric acid,UA)的相关性; 比较治疗后不同时间和不同疗效的急性发作期GA患者(Ⅱ、Ⅲ组)血清和滑液中IL-1β和COX-2的表达水平。结果 观察Ⅰ、Ⅱ、Ⅲ组患者IL-1β、COX-2和UA水平均显著高于对照组,且Ⅲ组显著高于Ⅱ组和Ⅰ组,Ⅱ组显著高于Ⅰ组; IL-1β、COX-2与UA均呈显著正相关(P<0.05); 入院时和治疗3 d、7 d、10 d时,Ⅲ组血清和滑液中IL-1β、COX-2均显著高于Ⅱ组(P<0.05); 与显效组相比,非显效组治疗14 d时血清和滑液中IL-1β和COX-2水平均显著高于显效组(P<0.05)。结论 GA患者血清和滑液中IL-1β、COX-2水平均显著高于健康对照群体,并与患者疾病严重程度有关,对二者的监测有助于临床疗效的评估。
Abstract:
Objective To investigate the dynamic changes and clinical significance of interleukin-1β(IL-1β)and cyclooxygenase-2(COX-2)in serum and synovial fluid in gouty arthritis(GA).Methods From January 2016 to December 2019, 80 patients with GA who were outpatient or hospitalized in Xuzhou Hospital of Traditional Chinese Medicine were selected as the observation group, including 29 cases in remission stage(group Ⅰ), 31 cases in mild stage(group Ⅱ)and 20 cases in severe stage(group Ⅲ), and another 30 healthy volunteers as the control group. The routine laboratory indexes and serum IL-1β and COX-2 in each group were compared. Spearman correlation analysis between IL-1β, COX-2 and uric acid(UA)was performed. The expression levels of IL-1β and COX-2 in serum and synovial fluid of patients with acute onset GA at different time and different efficacy after treatment were compared(group Ⅱ and group Ⅲ).Results The levels of IL-1β, COX-2 and UA in patients in group Ⅰ, Ⅱ and Ⅲ were significantly higher than those in the control group, and group Ⅲ was significantly higher than that in groups Ⅱ and Ⅰ, and group Ⅱ was significantly higher than that in group Ⅰ; IL-1β, COX-2 were significantly positively correlated with UA level(P<0.05); at admission, and treatment for 3 d, 7 d and 10 d, the serum and synovial fluid IL-1β and COX-2 in group Ⅲ were significantly higher than those in group Ⅱ(P<0.05); the levels of IL-1β and COX-2 in serum and synovial fluid of the non-significant group were significantly higher than those of the marked effect group at 14 days(P<0.05).Conclusion The levels of IL-1β and COX-2 in serum and synovial fluid of GA patients are significantly increased and related to the severity of disease. The level of IL-1β and COX-2 contributes to the evaluation of clinical efficacy.
