摘要:
下肢动脉粥样硬化性病变(LEAD)是糖尿病的常见慢性并发症之一,最常累及下肢动脉的远端血管。踝肱指数、彩色多普勒超声等是糖尿病LEAD简单、可靠、常用的筛查方法,可以达到早期诊断的效果。基础治疗、运动治疗、扩血管药物的应用等早期治疗能有效缓解症状,降低糖尿病足和截肢的风险。对于有明显的临床症状及体征的糖尿病LEAD患者还需借助CT血管成像或磁共振血管成像及数字减影血管造影检查,以进一步评价下肢血管病变的严重程度及病变部位,在基础治疗的前提下,联合血管重建及干细胞移植。
Abstract:
Lower extremity atherosclerotic disease(LEAD),which most frequently affects the distal blood vessels of lower limb,is one of the common chronic complications in patients with diabetes mellitus. Ankle-brachial index(ABI)and color doppler ultrasound are common, simple and reliable methods for screening LEAD in patients with diabetes mellitus, and can achieve the effects of early diagnosis of LEAD. The early treatments, including basic, exercise and vasodilators therapy can effectively relieve symptoms, reduce the risk of diabetic foot and amputation. For patients with diabetes mellitus and obvious clinical symptoms and signs of LEAD, it can define the area of vascular lesions and severity through computed tomography angiography, magnetic resonance angiography or distal subtraction angiography. On the premise of basic treatment,the patients with diabetes mellitus and LEAD get more benefit through revsculrization or stem cell transplantation.
